Connected Care is a local, digital shared care record for health and care professionals across the Frimley Health and Care Integrated Care System (ICS). It allows the secure sharing of your health and care data between authorised health and care professionals for the purposes of delivering safer, quicker, more personalised and better coordinated local health and care services.
Given its geographical coverage we will no longer be sharing your data directly with CHIE, a similar record covering the Hampshire area, although these records may be linked together in the future.
Benefits for your Care
This shared care record will avoid the need for you to repeat your story to different clinicians. Organisations using Connected Care will have access to the necessary information to better understand your circumstances and any services you have received resulting in fewer unnecessary conversations and duplication of tests.
Important details such as allergies, medications, diagnoses and treatments can be shared appropriately to inform the care you receive. Your clinician will therefore be able to see information from other organisations involved in your care.
By using secure electronic systems, your information is held safely, care details are kept up-to-date and accurate, and the risk of errors and delays to your care are reduced. Only staff with appropriate authorisation have access to your information and all access is logged.
An app is in an advanced stage of development which will enable you to access the information held in this record.
To help monitor and improve the quality of the services you receive your data is automatically processed in such a way to prevent identification by researchers. This enables our wider population data to be analysed in order to help find out how to improve the quality and effectiveness of our services.
Right to Object
You also have the right to object to sharing of your data via the Connected Care. However, the NHS strongly recommends the sharing of data and our experience demonstrates that it leads to higher quality and safer care. An objection to sharing your information will mean that your data will not be shared electronically for any kind of direct care, including extended hours GP access and emergencies. If you do wish to object, you should therefore contact your GP practice and the other organisations involved in your care and understand what it means for you.
Please be reassured that if you have already opted out from sharing your information it will not be shared with Connected Care. If, having previously opted out, you would like to opt back in please contact the surgery.
